I would consider a substructure tap the metal and do over crowns screw retained. Now you’re showing what I see to restore frequently.
It seems the surgeon was trying to avoid the sinus regarding the positioning of #3. Maybe because the patient rejected a sinus lift or the surgeon just assumed they would. Have you considered getting involved in the planning process and reverse engineer the case from the desired results down to the placement of the implants?
It doesn't eliminate all goofy angled implants, but at least you will know what you are faced with before the implants are placed. This also gives the patient the opportunity to decide ahead of the surgery if they are willing to make a compromise in aesthetics due to anatomic circumstances.
