3m Lava Form puck holder

Does anyone have any info on the 98mm puck holder for the original Lava Form machines? I have seen the puck holders for the old machines mentioned a few times but have never seen one. Thanks in advance.

The only thing i have heard of people doing is taking the Lava holder and having a machine shop make the same holder but with a 98mm insert instead of the Lava insert. Then they hire a IT guy to redo your software.
