For ten years I have never seen anyone mimic the caribelli cusp on upper first molars. Even when the upper first moler on the adjacent arch has a big fat prominant caribelli.
I am asking...
Do you build the extra cusp into your upper first when applicable?
Of course not speaking of expecting that on a glidewell crown, but on a more natural and proper crown.

We always match whatever is on the opposite side of the arch

I also always copy the opposite side of the arch. If there's only a quarter arch to work with, I'll add a caribelli.

Yup, if the case calls for it. That's the stuff that will set your lab apart from the assembly line labs.

I NEVER put them on my crowns if I can avoid it! They are stain & plaque collectors !

We're only putting it if the dentist is asking for it, and even then is just a small feature nothing extreme. Patients tend to complain of discomfort when they're getting it...

Yes, if they have one on the other side, we will put one on the crown/bridge.

I also add one when applicable. I do generally keep it small. Even if the opposite one is large.

What I find interesting about this is you will see a bunch more of the 5th cusp because of the dominance of cad designs. Many of the initial proposals have them.

I love to see gold crowns with them, I think they look sexy!

The patients going to be feeling the crown with their tounge to begin with; even if they have the development on their natural teeth they likely dont know it. Just some thing for them to 'notice'...and complain about.

Eh? If they had it on their natural tooth, they wouldn't notice it if you put it on the crown.

I have them on both my natural upper first molars... If I had one that had a smooth lingual and one that didn't you better believe I would notice it.
