i wouldn't want it to be the screw. drilling and tapping in that tiny space i would not be happy.

I wouldn't want to be drilling and tapping either, but I believe that the established protocol for failure of the screw is warranted. As stated before, that particular design may further reduce hard tissue trauma as in the case if an accident. So its more of a safety net, an asset rather than a liability. In the old days patients had recalls for maintenance of the restoration and that included placing new screws. We've gotten away from that protocol. Most screw breakage I have seen has happened on single implant restorations, a few on splinted: 3-4 unit bridges and were more than likely due to a non passive fit which over time lead to failure of the screw caused by cyclic fatigue.

Dr just called and said the head broke off the 3.0 Nobel.

He only noticed the warning card after he torqued past 15 Newtons.

So....what's the chance that screw is retrievable? Is there any technique to remove it do you wait for it to loosen?

If he cross threaded it going in, good luck getting it out. If the head is just broken off it shouldn't be under any tension and should back out easily.

He went past 15 newtons so it threaded properly. I think the pt went home but I emailed him my brilliant idea;)

A trick for removing broken screws is too use the tip of a ultrasonic cleaner and try to vibrate the screw out. Normally, but not always the weak spot of the screw is right below the head, which makes for easy removal. Unfortuneatly not all systems are designed that way.

The sick irony is that these are made by the same implant companies that cry foul when you use a 3rd party component.
