Hybrid Implant Abutment debonding

Hi everyone,
I had a Hybrid Implant case debonding today, after hundreds of successful cases using multilink implant (hybrid) I wonder What could have been the issue this time?

1. Cement went bad, exp date is in NOV/2014
2. I did not light cure enough time. ( we use 30 sec )
3. Contamination inside the abutment, I forgot to sandblast?
4. I should switch to one piece TI abutment before is too late?
5. any other suggestion is welcome, thank you

Do you use short titanium bases?
If you have only had one, probably a fluke-one of the things you mentioned.

Did you cut any retention grooves into the Ti base ?

I light cure everything for at least one minute however I'm sure mulitlink is dual cure.

Emax or zir?

Great tip Z Z, thank you, I will also be adding extra light curing time. We use short TI bases provided by B&D and only ZR abutments.

Correct, multilink implant is dual cure.

H2O not completely dry?
Oil from fingers touching bonding surfaces?
Excess Monobond not blown off?
Flawed automix tip?
Restoration get moved on the Ti base during light cure before the cure was complete?

And yes, I add at least one retention groove too.

If you've done hundreds, odds are you have your ducks in a row and it was a fluke, but it never hurts to recheck the basics. Good luck!
