Looking at this case I wonder if custom abutments with cement retained crns would be the answer? I also agree about the buccal-lingual occlusal table should be reduced. Especially because the occlusal forces are not being directed down the long axis of the implant. A cement retained solution may have made this possible. Having said that sometimes a picture can be deceiving.
