a few, more than a few, years ago a local dentist dropped an implant driver down the throat of a 90 year old patient patient. Off to the hospital where it was found and removed. At a later appt another driver was dropped, this time into the lung of the patient. Patient died as a result of this guys lack of procedural protocols, no floss tied to the driver.
I know this guy and this didn't surprise me.
