Need help identifying this implant

So story is Dr sent us a bio H 3.0 internal impression coping. We fabricate using correct components. Dr says its too big and doesn't seat. I'm thinking that the dr charted this incorrect and placed a different implant. Any idea what this is?
1pic is intra oral pic of top of implant connection. (to me this doesn't look like BIO H 3.0?
2pic is attempted delivery.

Implant is probably a goner with the threads exposed like that.

Doubt that is a tissue level implant.

Maybe the contacts were to tight? Did you mill it or is it a stock abutment? Did you have a model to check the fit? Do you have a pic of the abutment?

OS or GD placing implant is required by law to keep record. If this is second hand, he should professionally call the first professional.
You made on what he sent, so this is not a "remake".
