Does anyone do custom shades at the doc's office? How much do you charge?

We don't offer in office shades, instead we have a nice setup with proper lighting and chair in the lab for free. It works out well for both parties as we aren't sending a tech out all over to do shades and patients are willing to take the drive due to no cost to them.

I do the opposite of everyone else.
I have plenty of time to see patients at any office I work with. I also have the use of a private boardroom at 11 different locations across the city conveniently located close to the patients. This works so well for me and the patients.

I agree - I always see the patient in office so it’s minimum of an hour and a half away from the bench. It’s a huge inconvenience for a small fee - I try to get them to consolidate the appointments so I can see 3 or 4 in one shot. I’m not as concerned about the fee as much as I am the time savings - every hour I’m away from the lab is an hour later I have to work that day.
ouch, that can add up real fast!

Private boardrooms? You Canadians use odd terms, down here we call those bars.

We average 1 per day with the occasional busy day with 2 or 3. With our setup they come through a side door to avoid walking through the lab, and have a doorbell that goes directly to the ceramic manager to let him know. Works out pretty well having everything you need ready at all times in there taking only 10-15 minutes per appointment.

that isnt too bad. we have weeks were its been a dozen or more shades (and we are M-Th). it gets to be really difficult to get anything done when every few minutes theres a new patient walking in haha

Then you second guess yourself the minute they walk out the door after you take a breath..

The problem I have is taking the 'liability' if you can call it that, of being the one who has to determine an obviously tough shade. 2 people can look at the same Pt and one says a3.5, the other says c3. Whether it matches all comes down to when its seated and we dont have that luxury. Ive held crowns up and had the pt approve, many times, only to have it come back.
The real shade check should be at the try-in, and most Drs arent willing to go through that process, yet expect perfect results in 1 shot. This job.Banghead

Another thing to mention is having a system in place so that Drs arent just punting, but are still invested in the outcome.. I make sure that they give me at least their best guess, the closest range, pics!, something to get the case started so I have an actual crown to compare and adjust when I see them, often if its a tough shade or a central I end up having to see them 2x to start the case and check the final glaze..

I agree taking “full responsibility” for a very tough shade is the short end of the stick - I don’t think it’s completely fair to put the bag in the techs lap to match a really tough shade.. I’ve had some doozies in the past I remade 3 and 4 times until I got it right. It reminds me of a recent surgery my wife had - large cell tumor in her index finger .. surgeon took it out last year, it was a tough case for him since it was not encased at all and he couldn’t be sure he got it all out - 6 months later it was back and he had to do the surgery for the second time - yes he got paid full price for missing some cells the first time. Sometimes a case is really hard and you do your best and still miss it, our industry has a few head scratchers to it and this is one of them for me.

From an outsider the shade system seems too limited to begin with. When you consider the range of colors/shades the eye can discern the dental shade systems seem abbreviated.

You're right. I have 5 guides I always bring with me. If you think a vita classic can match a natural tooth you'd be wrong 95% of the time.

Yep, as for me its the Classical guide, the 3D guide, Halo Value Minus, Inc tabs and the emax press HT guide are the ones that pretty much cover all the shades for me. Besides InSync touch up at the bench when reviewing the photos. The HT had a bit to much Chroma, but I adjusted for that. Time to clean up my Inc tab ;) :)

I hate teeth. I wish I could be olivier tric and just make the patients wait around the lab all day while I get the 'feeling' of the shade, and charge $1000 a unit for it.

We charge $45.00 in lab and never go out to take a shade as this is not profitable in any way, plus how far do you drive and how do you get your own work done?
