He needs to be honest with the patient and let them know that this is a tough case and he is going to refer you to a specialist that handles hard cases like this on a daily basis.
This is another one where the OS needs to be referred somewhere too. If 'that's where the bone was' then the bone isn't where it needs to be, fill it and wait, or don't drill.

Make it a two unit? with the access hole on the lingual between the two units and the support for porcelain split on either side. leaving a bitch'n diastema for spitting.... or have the doc prep number 25?... or punt... idk, man. This one is over my head.

this case reminds me of a popular Phrase people who install drywall say " fix it in mud ".

The GP/OS said " fix it in the lab". You have been set up for failure and when you do, well, you probably lost an account.

Isn't modern dentistry great.
Lets rip a few thousand out of somebody's ass, then send it to somebody else who makes the least off of it to "figure out how to make it work".

"I can't tell the patient the OS must have been drunk when he placed the implant and charged them $2000, that would be unethical. Here figure out some scheme to save our 1 hour of work. Ask your internet friends for help".

@Brett Hansen CDT Do you remember what happened with this?
I created a UCLA abutment and then we made a 3 unit cantilevered bridge. As far as I know, it is still in the patient's mouth. I relayed a lot of the responses from this thread to the doctor at the time. :)

Every time I hear 'Do your best' I wonder if that's becsuse they think I don't normally or if they know they did not.

It's like someone asks you to scratch an itch on their back, and then says "don't scratch too hard". Well, does that mean I am scratching too hard or that you just don't think I know scratching too hard is bad?".

I could put lingual wings on to the two teeth adjacent to the restoration site.
It is never recommended to link natural teeth to implant, and I hope you know the reason why. Maryland bridges or wings were never successful in the first place. I would not do this case. It is a failure waiting to happen. As far as the collar on the abutment, I always trim mine to below the gum line even with stock ones. Good luck.
