I'm an in house tech in a perio practice, I use Tak Model Bloc for blocking out for these. It's easy to adapt, stays in place, easy to remove, and doesn't melt.
I just tried the links on two different computers as well as my iPad and my Samsung Note 4, it worked every time. They are semi large files for a PDF (46MB) so it may take a few minutes for it to load.
Hmmm. It's been a long time since I installed it. Iirc it took me a while to figure out how to make it work correctly. I'll have to mess around with it when I get some free time.
If you have Cerec v4.2 you can export the file as a .cdt then import it into Blender v2.63 using the CDT Total Importer then export it as a .STL

Here’s a full mouth case that I am in the middle of. The vertical was opened 2mm. So far we have completed the upper and lower anterior sections using A1 e.max multi that have been facially layered with e.max Ceram. We pressed all of these with JP Vest in my Ivoclar EP5000.
They need more bleach shades in the Multi ingot line!
Also, I updated my EP5000 to the 7.1 software yesterday and now I am having issues pressing the multi ingot.
