I received this from a doctor of ours, any help would be appreciated:

Dr. Michael Murphy, recently deceased, did the implant body. His records are not at all helpful in what exactly he placed there. It is time for a new crown and I can not figure out what the attachment is. It is a screw retained porcelain to metal crown.

The problem is that instead of a recess in the implant body, there is a raised hexagonal platform that fits into the recessed crown---all backwards to the usual.

I have tried to get information about any kind of hexagonal attachment that might have been used, but no one knows (or is willing to research) this situation.

I have enclosed some pictures of the crown and the implant body/attachment that better illustrates this situation. Any ideas, knowledge, that you can throw m 20170627_154001_1506358361311_resized.jpg 20170627_155614_1506358355045_resized.jpg 20170627_155719_1506358355735_resized.jpg y way would be greatly appreciated.

I did a case like that a year or two ago I think? Maybe a Branemark external hex or something like that? I think I got a plastic castable abutment from Attachments International? AI is part of Implant Direct.

This is a Noble Branemark/Speedy. Were you able to get a closer look? Does this match up?

Doesn't look like a Branemark. Must be something else. Attached image is of a Branemark.

Get the X-ray if you can. I would peg this as a Octa abutment on a Straumann or maybe Lifecore One

Here's what the Doctor sent:
I did some looking and found two radiographs of # 9. However, both of them are with the crown cemented in place. I have no others. Additionally, the original info from the oral surgeon said that it was :

Straumann
SL active implant
4.1 mm RN x 12mm

That has proved to be no help for me. Maybe it will ring a bell for you.

It's a Straumann synocta tissue level implant. Pg. 83 in the 2017 catalog

You are in luck then. Straumann still offers parts for this. The Octa abutment comes in two flavors depending on how long ago this was placed. Original Octa abutment was 2.5mm high, the later version is 1.5mm high. You can create the same type of restoration as the old one with a UCLA or you could ask the Dr to take off the Octa abutment and take a fixture level impression for a cementable restoration.
