Be careful, the Dr. has you in house for a reason and you are a big draw no other dentist has your work currently. If other Drs. can sell your work it may become a conflict of interest.

1-DBA
2-zoning check
3-business license
4-IRS number
5-insurance.

In other words, get yourself a halfway decent accountant and she/he will do all this for you...even if you want to sign-up as a sole-propreitor versus the tax benefits of doing a S-corp.

pretty much. See if you can get the IRS number 1st, it'll most likely be needed on your zoning and business lic stuff. They want to know who to fine.
Last time I filed for an LLC I was able to file and obtain a FEIN then and there.

I also would put a good accountant near the top of the list only secondary to a business plan. The accountant will guide you through most if not all that has been mentioned and set you up with a good explanations of accounting practices and procedures that will keep you squeaky clean with the taxes.
