I cant rotate my laptop screen to read what it is you posted. I am guessing postal mail of some sort. Is it for your shipping setup? I would like to find out what smaller labs preferred shipping methods are. I called dhl today to send something to NY for a second day envelope, 23.00 The USPS is 6.00 but take 3-4 days.

It's a 3615 form. Used to obtain a Business Reply Mail account with the post office. Print it out if youneed one, or got to the post office.
I have a BRM for certain accounts, I use FedEx for everything else.
My BRM is set up for incoming shippments. I also have Fedx set up for incoming as well. Just depends on the turn around time factor.
The BRM I have costs $180.00 per year. I put money in that account that is used up as mail comes in.

What I tell the Dentists is if they will mail in on their dime, we'll mail it back free. Just incorporate the shipping cost into the total price of the appliance.

Yup...we pay postage one way generally, there are exceptions. Like maybe they want it quick. Some just like to use a prepaid mailing label for USPS. Some like a label from FedEx. What ever they want.
I put this 3615 up for "forthem" actually. She wants to set one up for study model shipping. For that particular thing, a BRM works great for study models (not much else) and the offices seem to prefer it for cost etc.

Yep, thats what I do, I use UPS.
Ill never use DHL, and I told all my suppliers never to ship anything to me DHL. Next day air to DHL means 2 to 3 days and their 2nd day takes a week.

I offer shipping only one way but if they want to use a prepaid label then I print one out for UPS and then charge them. I have started to use the post office for shipping to some of my accounts. Costs me $4.80 to ship priority mail compared to over $6.00 for UPS and the packages will get there the same time for these accounts.

DHL no longer offers service in my area. I thought it was funny with them running all these commercials lately but they have cut back the service they offer.

Default USPS Shipping

I found some shipping info that some of you might be interested in.

On the USPS site https://sss-web.usps.com/cns/landing.do you can set up a business shipping account with a credit card and pay less than the P.O. counter rates. It allows you to print a label with postage on it. A 1 pound Priority package is $4.75 instead of $4.80 and larger packages are up to 11% discount. A nickel isn't much but if you ship very much it could add up. It could also save driving to the Post Office.
